In this assignment, you will review a patient scenario in iHuman. Please see the course announcement for the case assigned this week. Your deliverable for this assignment is a paper based on the iHuman Soap Note Template (Word).

What is your full name, DOB, and where are we located (town or address)? Do you share the same concerns about your memory as your daughter? If so, please describe. My name is Selah Martin. My birthday is 1/28/1953. We are in Layton, Utah. My memory is fine, my daughter just worries about me. I forget things sometimes, but I am getting old.

*Britt: how long have you noticed memory changes? When was the last concussion? Her last concussion was 2 months ago. She slipped down the front porch steps on some ice and hit her head on the railing trying to catch herself. She got pretty banged up. I took her to the ER, but they said it was just a mild concussion. She has seemed more tired lately, and she has trouble finding the words, and she sometimes calls me by the wrong name. She is always losing things. Sometimes its things she has had in the same place for forever.

*Do you have any headaches? If so please describe. Any light sensitivity? No headaches normally. Sometimes I have them but they do not bother me.

*Do you have chest pain, shortness of breath, dizziness or heart palpitations? None of those things.

*Any difficulty sleeping? Has there been a change to your sleep pattern since your concussion? I toss and turn a lot. I cannot seem to get comfortable and my bones ache.

*Are you sleeping more or less than usual or having difficulty falling asleep? I go to bed around 9pm and wake up at 6am, but I usually lay in bed until 8am. I just feel tired and do not want to get up yet.

*How is your mood? Do you find yourself becoming more irritable or sad? Have you noticed difficulty concentrating on things or feeling slow? I am a little sad. It is hard getting older, a lot of my friends and family have passed on. My children are busy with their families. I do notice it getting harder to concentrate, but I move slow because of my arthritis.

*Other than the forgetfulness, have you noticed a decline in cognition, langue or mobility? I don’t think so. But I definitely dont move like I use to.

*Do you have difficulty with speech or other forms of communication such as using the phone? I dont think anyone has troubles understanding me. Sometimes I don’t know what things are called but the person can usually help me with that.

*HTN: When were you dx? What medications are you taking? Any recent changes? Do you check your BP at home, readings? I have had high pressures for a long time. I take lisinopril 5mg every morning, and metoprolol for heart. I have times where my heart goes real fast if I don’t take it. I check my BP at the Walgreens by my house. My daughter takes me there to pick up my medications and I check it. Those machines hurt my arm.

*DM: When were you dx? What medications are you taking? Any recent changes? Do you check your BS at home, readings? What was your last A1c? Do you see an endocrinologist? Oh I Don’t know. I cant remember what age. I don’t think I am really diabetic. My daughter makes me check them but they’re rarely high, and I dont take my medicine every day. What an A1C? My daughter might know.

*Hyperlipidemia: When were you dx? What medications are you taking? Any recent changes? Have you had any recent labs? I am taking fish oil. I took a regular medication to help with my cholesterol but it hurt my joints alot. My regular doctor told me to stop it. I think he checked my cholesterol and said it wasnt too high.

*TIA: When were you dx? What medications are you taking? Any recent changes? Do you see a specialist? Are you taking a daily aspirin or blood thinner? Have you noticed any signs of bleeding, dark stools, bruising? I had a TIA last year. I had troubles speaking. I knew what I wanted to say but couldn’t get the words out. My daughter said I was talking but I didnt make sense. I don’t remember a whole lot after that. Yes, the doctor put my on daily aspirin and I am on Xarelto. Thats why I have all these bruises everywhere.

*Osteoarthritis: Where is your knee pain? How long has this been occurring? Please describe the pain, what is the severity, 0-10 level? What activates make your knee pain worse? What activities make your knee pain better? Do you see an Orthopedist? What medications are you taking for pain management? Both my knees hurt. I was a gymnast when I was little and it banged up my knees. I have osteoarthritis, but I don’t know when it started. Most days my pain is maybe a 3/10? Walking around for a long time makes them hurt real bad.

*Falls: Please tell me about your recent falls. How did you fall, was it a trip or did you feel lightheaded or weak before your fall? Did you see medical attention for these falls? Were you hospitalized? Do you see a Neurologist? I feel down the front steps and hit railing with my head. They were icy from water dripping off the roof. I went to the ER because my daughter made me since I am on the blood thinner. But, they did a scan and said I was fine and sent me home.

*Other than what you have told me for medications above, are there any other prescription or over the counter medications, vitamins or supplements you take? Naproxen and Voltaren for my joints.

*Do you have any drug allergies? Sulfa.

*Who lives with you at home? Do you feel safe in your home? Do you wear a life alert or carry a cell phone? Do you use any assisted devices for walking? Do you drive? Do you need assistance with ADLs? My daughter lives with me. She takes good care of me. I do not walk with anything. I don’t drive, people are crazy! I don’t need any help with washing. My daughter is here to help me.

*Do you use tobacco products, if in the past, when? Do you drink alcohol, if so, how much daily/weekly? When I was real young I smoked occasionally with friends but haven’t touched one in 50 years. I drink an occasional beer. may one every other day.
